Access the Patient Portal
The SOSI Patient Portal is a special secure and confidential web-based tool to help engage patients and families in their care and accessing your health information. The Patient Portal will allow you to communicate with our office and your care team staff quickly and easily, in a secure manner.
New Account Registration
By enrolling in the patient portal you will have easy online access to:
- Complete registration forms prior to your appointment
- Send messages to your care team
- View your medications, lab results, allergy list, and medical history
- Submit an appointment request, review upcoming appointments
- Health library
To create an account, you will need a “PIN” provided to you by our office. A PIN is a unique number that we email to you to grant access to create your account. If you have not received a PIN by email please contact our office at (970) 879-6663.
After receiving your PIN, visit Patient Portal to create your account by clicking the "Create Account With PIN" button. Once in the portal, click on the Let's Get Started Button and follow the prompts to finish setting up your account.
Once you have an account you can log-in directly from this website in the future.
